Service summary

Supporting people to lead independent and active lives in their community Cornerstone’s Aberdeen City Outreach service specialises in taking a personalised approach to the delivery of social care & housing support. We are able to support adults of all ages presenting with a diverse range of needs. This includes but is not exclusive to supporting individuals presenting with learning disability, physical disabilities and mental health issues. Our first step is always to collaborate with the person requiring support and people who are important in their lives to establish what things are most important to that individual. Following on from this we work with the individual to develop a personal support plan that will reflect their personal interests and ambitions. Our aim is to support each individual to attain optimum independence, to get the most from their lives and be regarded by others as valued members of the community. Cornerstone have demonstrated in the past an ability to make a significant difference not only to the people whom we are directly supporting, but also to their families and carers. Support provided can range from a few hours a day to 24 hour support. Cornerstone promotes a culture of staff development, encouraging our staff to expand their skills through continuous training. We strive to work at all times in accordance with the SSSC Codes of Practice and the National Care Standards. All our staff undergo PVG checks prior to taking up their posts. Our Outreach services can provide: • Social and Recreational Activities – facilitating access to a wide range of opportunities such as social clubs and disco's. • Education and Employment – facilitating engagement with further education, training and employment opportunities. • Budgeting - providing support to enable people to manage their finances. • Tenancy maintenance - support can also be given to help people with all aspects of tenancy maintenance, including general housework, support with medication and the preparation of meals.
