Cornerstone SDS: Self-Directed Support - Helping you to choose the best possible care and support Cornerstone SDS is an impartial advisory service, created to provide you with all the information you need to choose the best possible care for you through Self-Directed Support (SDS). Within the Cornerstone SDS Team, we have a comprehensive knowledge of the social care sector and a wide range of skills to help you think about the development of natural supports as well as paid support. Whether it is to answer a question or to assist with the entire SDS process, Cornerstone SDS is there to help you make the choices that are right for you. Our services include: SDS Advisors who can: Empower you to make informed choices about the SDS options Provide advice and support to help you employ your own Personal Assistant if required. Support you to manage Direct Payments if required. • Liaise with family members or professionals such as care managers and social workers. ISF Coordinators who will: Meet with you face to face to discuss your personal plan Co-ordinate and book all necessary provision. Hold and manage the budget on your behalf. • Ensure money is spent on meeting your outcomes as agreed in your personal plan. Resource mapping/Signposting: We have community resource and research coordinators who will signpost you to services, groups, organisations that are available locally to help you lead a good life. You don’t need to be eligible for SDS to use this service. Training/Awareness Raising: The Cornerstone SDS Team are dedicated to raising awareness about SDS and will visit groups, staff teams and organisations to share information and knowledge using formal and informal methods.
